Sometimes tamariki need extra support transitioning from ECE to school. The flowchart below indicates the types of transition support that may be available. RTLB support the middle example, where there has been an Early Intervention teacher involved at the ECE. More information and the referral form are further down this page.
EIT completes RTLB referral (below) and obtains consent
EIT emails this to Cluster Manager and School SENCo
EIT sets initial meeting date and invites all team members
EIT facilitates initial meeting 6 weeks prior to school start
Team makes use of learning partnership questions in the Project form
RTLB documents meeting notes and shares with team
Team develops plan and implements
RTLB attends one or more child-to-school transition visits
RTLB becomes key contact for school once child starts school
RTLB will ensure regular visits are made and data/observations are collected
RTLB supports school with identifying professional development and resourcing needs
EIT shares resources and information to assist with transition
EIT is available to liaise with RTLB and school
EIT provides 1-off assessment if needed.
RTLB sets review meeting date and invites all team members
RTLB facilitates review meeting 6 weeks after school start
Decide on next steps and if a full Request for Support is needed

We can support students who have current or recent (within the year) RTLB Requests for Support (RfS).
Request for transition support arises from discussion between current school SENCo and Liaison RTLB prior to transition and/or as a result of CAP or review goals.
Transition may be supported within an existing RfS or through a new Individual or Group Transition RfS. If applicable, a new RfS completed on the database by RTLB liaison and current school SENCO.
Previous RTLB RfS may also be supported less formally through the liaison rather than through a separate transition RfS.
Permission should be gained from whānau at Review meeting or through the SENCo at a later date.
Liaison RTLB may also support the school’s regular transition procedures through SENCo/liaison hui.
Transitions from Primary to Intermediate and Intermediate to Secondary are a key focus. RTLB play a role in ensuring information gets passed from school to school (ensure consent is gained).
When a learner moves from one cluster to another, clusters work collaboratively to ensure smooth transition processes.
